Transitional Living Program

The need: The Transitional Living Program (TLP) at Christ’s Home began in 2004 in response to a community need for a program to serve older youth in foster care transitioning to adulthood in a safe, supportive, and empowering environment.

Statistically, older youth in care face a higher risk of poverty, homelessness, addiction, and criminal records. These risk factors contribute to outcomes of inconsistent education opportunities, life skills training, and employment opportunities.

What it is: The TLP program is centered on recognizing and healing from the past while teaching youth to grow into their futures. Youth in the program build meaningful connections and skills that will help guide them to actively participate in the planning and direction of their futures. The TLP Program is licensed with the Pennsylvania Office of Children, Youth, and Families (OCYF) that supports youth who are transitioning to adulthood from foster care and other placement settings. While in the program, youth receive high quality trauma-informed care and services that support their strengths and build their skill sets. Christ’s Home recognizes the broad impacts of trauma and the unique ways trauma impacts older youth.

 

TLP is centered around seven core program tenants:

  • Trauma-informed care
  • Physical site safety
  • Meaningful family involvement
  • Meaningful youth involvement
  • Visitation
  • Youth skill development
  • Enhanced staff training
  • Team discharge planning

 

Program Goals

  • Assist youth in learning essential life skills, including home and money management, self-care, education, employment, and accessing community resources, so that they may be self-sufficient once they are released from the foster care system.
  • Help youth to identify attitudes regarding living independently and help build self-nurturance, self-confidence, and an ability to maintain healthy relationships.
  • Assist youth in establishing meaningful connections with support networks outside of the program.
